Description:

As an LPN /RN, you will be responsible for providing quality care under the supervision of a Registered Nurse (RN) to meet the varied needs of our residents/patients.
  • The LPN/RN is responsible for initial data collection on all residents.
  • The LPN/RN is responsible for utilizing the principles of Relationship Based Care while performing duties.
  • Implementing person driven care plans as developed by nurse management.

Qualifications:

  • Current MN Nurse licensure , in good standing.
  • Must be able to read, write and communicate in English.
  • Clinical experience preferred.

Description:

Certified Nursing Assistants (CNA) are amazing and dedicated professionals that provide high-quality, compassionate care to our residents daily. As a CNA you operate on the front lines and ensure residents/patients are treated with dignity and respect, as well as choices within their lives. The CNA provides direct resident care and report condition changes to RN or LPN while also charting in POC system.
  • The CNA demonstrates time management and prioritization of tasks.
  • Must be in good standing on the State Nursing Assistant registry or appropriate entity
  • Must have good communication skills
  • Must be able to read, write and communicate in English

Qualifications:

  • Must have completed a state approved CNA Program
  • Passed the state approved CNA competency exam
  • 64 hours completing the 7 modules of Health Care Core, 40 hours of lab instruction and 24 hours of supervised practical training with a live person in a lab or other setting
